Part-Time On-Call RN – Home Health & Hospice

Location : Northern Utah (Logan & Brigham areas)

Schedule : On-call 1 week per month - Monday–Friday 5 : 00 PM–9 : 00 AM, and 24 hours on Saturday & Sunday

Position Overview

As an On-Call RN , you will provide after-hours triage, nursing visits, and support to patients and families in their homes. You'll collaborate closely with our interdisciplinary team to ensure continuity of care and timely response to patient needs during evenings, nights, and weekends.

Schedule

On-call 1 week per month

Monday–Friday : 5 : 00 PM – 9 : 00 AM

Saturday & Sunday : 24-hour coverage each day

Key Responsibilities

Provide skilled nursing visits for urgent needs, admissions, or symptom management after hours.

Respond promptly to patient and family calls to provide triage and support.

Communicate effectively with physicians, team members, and oncoming staff.

Maintain accurate and timely documentation in the EMR.

Deliver compassionate care consistent with Ascend Health's mission and values.

Qualifications

Current Registered Nurse (RN) license in the state of Utah.

Minimum of 1 year of nursing experience (Home Health or Hospice experience preferred).

Strong clinical assessment and communication skills.

Reliable transportation and a valid driver's license.

Ability to work independently and manage time effectively.
