Description :
Provide physical therapy services to patients in their place of residence.
Patients may have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
Patients’ ages can range from birth to over 100 years.
Collaborate with patients, families, physicians, and other health professionals to identify patient needs.
Coordinate services to meet the identified needs.
Utilize thorough and timely electronic documentation.
Engage in interdisciplinary communications in accordance with agency policies and standards of practice.
