DESCRIPTION :

The Nurse Practitioner promotes and maintains health by providing medical services under the supervision of a physician. Contributes to physician's effectiveness by identifying short-term and long-range patient care issues that must be addressed;

providing information and commentary pertinent to deliberations; recommending options and courses of action; implementing physician directives.

Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories.

Other duties as assigned by the Attending Physician and Clinical Director.

Hours : 8 hours per day, 5 days per week

ESSENTIAL FUNCTIONS AND DELIVERABLES :

  • Provides medical management and coordination of patients according to designated standardized procedures and in collaboration with the supervising physician as evidenced by : performing advanced age-specific clinical assessment / triage, care planning, treatment, interventions, and evaluation / monitoring;
  • identifying patients in early crisis; collaborating with the interdisciplinary healthcare team for early intervention and optimal patient outcomes;

documenting care and care planning legibly in the medical record.

Performing duties / designated procedures within Nurse Practitioner scope of practice and in accordance with medical center policies, protocols, techniques, guidelines, standardized procedures, and state and federal regulations;

participating in medical record review and data collection.

Serves as a liaison and resource in area of specialty for a culturally diverse multidisciplinary healthcare team, patient, and family populations as evidenced by providing patient / family education and support in complex clinical situations;

providing information to appropriate individuals regarding plan of care, e.g., hospital course, post-operative plan of care, expected outcomes, follow-up care.

  • Serving as a preceptor for students; providing training and teaching to students and multidisciplinary healthcare team members.
  • Participating in development, review, revision, and implementation of standards of care, standardized procedures, policies, protocols, techniques, guidelines, patient education materials, and documentation tools.
  • Participates in on-call rotation for designated area as needed.

QUALIFICATIONS :

Licensure / Certification : Active California Registered Nurse (RN) license and California Nurse Practitioner Certificate required.

Nat'l Provider Identifier (NPI) and DEA

  • License required. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • Education : Master's Degree and completion of NP curriculum required.
  • Experience : Minimum three (3) years of experience as a NP in a family practice or pediatric outpatient clinic and two (2) yrs RN in an acute care setting or in a sub-specialty clinic environment required.

Preferred :

  • FQHC or Community clinic experience
  • Bilingual, Spanish / English

Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
