Registered Nurse Clinical Intake Coordinator

A company is looking for a Full-Time Home Health Clinical Intake Coordinator Registered Nurse (RN).

Key Responsibilities

Gather necessary information from referral sources and promote agency services

Evaluate referrals for client appropriateness for home health services

Receive, document, and process medical orders and referrals in a timely manner

Required Qualifications, Training, and Education

5+ years of Registered Nurse (RN) experience

2+ years of home health RN experience

1-3 years of experience in customer service, homecare intake, case management, or admissions preferred

Graduate of an approved nursing education program

Current Registered Nurse licensure in the practicing state
