About the Role :

Join our Skilled Nursing Facility team as a Physical Therapist, providing specialized care to residents recovering from illness, injury, or surgery. Work closely with nursing staff and other therapists to support patient rehabilitation and improve quality of life.

Responsibilities :

Evaluate and provide physical therapy treatments to SNF residents.

Develop and implement individualized rehabilitation plans.

Monitor and document resident progress, adjusting treatment plans as needed.

Educate residents and families on mobility, safety, and functional strategies.

Collaborate with nursing and interdisciplinary staff to ensure coordinated care.

Requirements :

Licensed Physical Therapist in Arizona.

Strong communication and interpersonal skills.

Experience in SNF or post-acute care preferred.

Ability to work independently and as part of a care team.
