The Patient Associate (LPN) is a key member of the HaH operations team and is the 'glue' supporting all hospital-based logistics prior to HaH admission.
The LPN is a high-energy, patient-facing team member who coordinates and supports an exceptional patient / family experience during the HaH admission process.
The Patient Associate (LPN) utilizes nursing process to provide patient care under the supervision of the HaH Admitting MD / APP (the clinician responsible for screening, consenting, and admitting all HaH patients).
Performs duties and responsibilities in a manner consistent with Mercy HaH mission, values, and the service standard.
The Patient Associate (LPN) reports operationally to the HaH Director of Clinical Operations (DCO) who has day-to-day responsibility for HaH operations.

Qualifications :
Ability to self-organize and prioritize work in a fast-paced, matrix environment.
Computer skills : proficiency with Microsoft Office and Teams; more broadly computer savvy with a demonstrated ability to learn and navigate multiple systems.
Attentiveness to HIPAA Confidentiality and Privacy Policies.
Preferred Experience : 1 years' experience with patient access and delivery systems, and / or prior healthcare clinical operations experience strongly preferred.
