Under the direct supervision of a pharmacist, performs transition of care (TOC) related tasks such as collection and verification of allergy and medication history. The TOC liaison will communicate with patients, families, nurses, physicians, and other healthcare professionals to obtain accurate home medication lists upon hospital admission. Provides integrated support for patients and the hospital's admitting team to ensure a safe medication transition by providing a "hands-on" approach to total quality patient care. Major Responsibilities :

• Provides integrated support to patients, pharmacists, and various healthcare providers by providing a "hands-on" approach to total quality patient care.
• Works closely with patients and / or their families to obtain the best possible medication and allergy history. Includes :

Interviewing patients and / or their families

Experience / Skills : Required :

Preferred : Experience with medication history program preferred.Community and / or ambulatory pharmacy experience preferred.Schedule details : Future Saturday only weekend rotation.
