South Carolina Licensed Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and barriers to care, recommending plans for optimal outcomes

Develop and monitor care plans, collaborating with providers and community resources

Provide education and resource support to members and their families regarding healthcare options and benefits

Required Qualifications

Must reside in South Carolina

Current South Carolina RN license

2+ years of nursing experience

Behavioral health and case management experience preferred

Degree from an accredited school of nursing or a Bachelor's degree in Nursing
