Location : Remote, but candidates should live near Hendersonville, Nashville, TN.

Start Date : ASAP

Full Time

No Weekends

Negotiable Pay / Weekly Compensation

Duration / Schedule : 3 months with likely extensions

Patient Volume : Complete 3+ patient visits daily

Responsibilities

Maintain quality of care standards

Assist patients with enrolling in educational programs

Knowledge of disease diagnosis and prevention

Assess and evaluate patients' health status

Implement and follow up on care plans

Document patient information comprehensively and timely

Counsel and educate patients and families about health benefits and programs

Requirements

Experience : 2 years of direct patient care relating to polychronic conditions Polychronic conditions, also known as multiple chronic conditions (MCCs), refer to the presence of three or more chronic diseases in a single patient. These conditions can include but are not limited to diabetes, cardiovascular disease, cancer, and more.
