CASE COORDINATOR

Reports to : BI Program Manager

Job Summary : The Case Coordinator provides supervision to frontline BI staff and case management to families served in the BI Program.

Responsibilities

Case Management

• Family satisfaction : ensure families are given proper support, advocacy, resources, communication
• Complete required assessments with families, adapting to the needs of the client and family
• Attend court appointments + other professional family-based meetings
• Matching BIs to homes, resolving issues, attend team meetings
• Review Incident Reports, follow-up with family, if needed
• Facilitate Referral Triage, Intake & Meet & Greets for each family
• Submit approved Recommendation & Approval Letters for services
• Create Behavioral Plan for each client
• Provide updated communication during the Admissions & Hiring processes
• Complete Quarterly Meetings & Bi-annual Assessments with each family
• Write / submit renewals
• Update SAM, electronic health record, & profiles
• End of Month Reports
• Case Closure & Aftercare Follow-up meetings,

assessments, and documentation
