License : Must have current RN license

Must-Have : Proficiency in HomeCare HomeBase (HCHB) and OASIS

Job Description :

• Provides, coordinates, and directs the provision of home nursing care according to physicians' orders and based on agency policies and procedures through the competent application of the nursing process.
• Responsible directly to the Clinical Manager.
• Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conducts home visits and participates in periodic office meetings.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Assigned between 5-10 patients per day that equate to roughly 6-8 productivity points.
• Responsible for different types of cases including Start of Care, Wound Care, and Follow-up Visits.
• Given territory mileage radius of up to 75 miles from the home clinic; mileage is reimbursed at the IRS standard rate.
• Locals are accepted at the same rate.
• Required to participate in on-call rotations for both weekdays and weekends.
• Estimated travel is 30 / 40 miles a day, averaging 75+ miles daily, amounting to 300+ miles weekly.
• Must complete all patient documentation within 24 hours of the appointment.
• Weekly productivity expectations of 30 points which equate to approximately 40 hours per week.
• Potential for contract extension or hire pending performance review.
• Required to wear scrubs according to the dress code.
