CLINICAL CARE MANAGER

Full Time

Needed to provide clinical direction to community support staff in accordance with the person centered recovery plan and directing the activities of targeted Case Managers, Peer Support Specialist and Behavioral Health Specialist.

The Clinical Care Manager serves as the point of first contact for service entry and assumes responsibility for ensuring the smooth and orderly coordination of multidisciplinary services to Adult with severe and persistent mental illness and children with severe emotional disturbance.

• Position requires a Master's Degree in Social Work with Certification or Licensure already in process or approved by the state of KY.
