Summary : Provides psychosocial services to the residents at their assigned facilities and works with the facility Social Service Staff and Clinical Staff within their region to ensure daily resident care is implemented per company and regulatory guidelines. Assists with education, orientation and survey management for the region under the guidance and direction of the VP of Clinical Services and the Regional Social Services Consultant
Qualifications : Education :
BSSW, MSSW preferred.Licenses / Certification :
Full Licensing as required by the state of employmentExperience :
5 years of experience in Social Services in a long term care environmentJob Functions
Provides education, training and support to appropriate facility staff to facilitate improved resident care and outcomes associated with their essential function / dutiesPrepare facilities for issues and concerns during survey processProvides direct psychosocial intervention.Performs resident assessments at admission, upon condition change and / or annually.Creates, reviews and updates care plan and progress notes.Provides direct psychosocial intervention.Coordinates resident visits with outside services,dental, optical, etc.Assists resident's families in coping with skilled nursing placement, physical illness and disabilities of the resident, and the grieving process.Works with the patient, family and other team members to plan discharge.Conducts in-service programs to educate staff regarding psychosocial issues and patient rights.Performs other tasks as assigned.Conducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standardsKnowledge / Skills / Abilities :
