JOB TITLE : Community Health Caseworker

Reports to : Director of Community Engagement and Innovation

SUMMARY : The Community Health Caseworker position will provide direct care services including crisis intervention if necessary to Spectra Health patients. They will work within the integrated team to plan and implement programs to meet the social and emotional needs of patients and their families at Spectra Health. They will collaborate with team members to coordinate patient care within Spectra Health and with community agencies. Community Health Caseworker duties include assessing, planning, and implementing care plans for patients as well as monitoring and evaluating those cares plans to meet the patient's health and human service needs. The Community Health Caseworker reports to the Director of Community Engagement and Innovation.

PHYSICAL REQUIREMENTS : The Community Health Caseworker is a position that exists in both an office, clinic, and community setting. When in the office setting the job requires the ability to sit for long periods of time at a desk or workstation. When in a clinical setting there would be long periods of time on one's feet, standing and moving throughout the clinical environment. Periodically, the position requires lifting or carrying items that would be appropriate in an office environment, not more than 20lbs.
