Responsibilities :

Requirements :

Bachelors degree in nursing. Two (2) to five (5) years experience in oncology case management and / or utilization management preferred. Knowledge of the Patient-Centered Medical Home initiative. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patients status and provide care as described in the departments policies and procedures manual.

OR

Equivalent combination of education and experience.

Current Colorado RN license required.

Current BLS Certification
