About this Role

Are you a compassionate and skilled Registered Nurse looking for a role where you can truly make a difference? As an RN in Complex Care, youll work in a collaborative, multidisciplinary team, using your clinical expertise and compassionate approach to create meaningful impacts in the lives of our participants and their families.

What does success look like in this role?

• Collaborative Spirit : Actively participate in the Interdisciplinary Team (IDT) by sharing valuable insights from assessments, contributing care plan recommendations, and ensuring seamless care coordination.
• Comprehensive Assessment Skills : Conduct face-to-face nursing assessments that encompass physical, psychosocial, and behavioral health, meeting participants where they arewhether at home, in our day center, virtually, or in a long-term care facility.
• Participant-Centered Care : Partner with medical providers to deliver personalized care for a dedicated panel of participants, ensuring their unique needs and care plans are prioritized.
• Clinical Excellence : Deliver nursing interventions with precision and care, accurately responding to physician orders, administering medications, and executing therapeutic interventions.
• Decisive Action : Use your triaging expertise in outpatient settings, initiating therapies within your scope of practice, and working with medical providers to escalate care as needed.
• Proactive Case Management : Navigate transitions of care with ease by maintaining strong communication with hospital and long-term care facility case managers to ensure continuity and collaboration.
• Effective Educator : Empower participants, their families, and team members by educating them on how to implement and sustain personalized care plans with confidence.
• Detail-Oriented Professionalism : Maintain accurate, timely documentation of all participant interactions and clinical interventions in the electronic medical record (EMR), adhering to regulatory guidelines from DHCS and CMS.
• Crisis Response Leader : Respond swiftly to medical emergencies, coordinating with external providers and family members to provide effective care in urgent situations.
• Team Player : Step in to support the team as needed, contributing your skills and expertise in ways that make a meaningful difference.

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
