Mercy Clinic 65 Prime Plus - North County 637 Dunn Schedule : Monday - Friday, 0800-1630No weekends or holidays! Mercy 65 Prime+ Clinics are a new primary care service designed specifically for Medicare beneficiaries. Patients receive extended office visit times and closer monitoring of chronic conditions like high blood pressure, diabetes, and heart disease. As a Primary Care Nurse (PCN) in this clinic, you will work closely with the providers to triage incoming calls / messages, educate patients, follow-up with patients, and assist with various clinic tasks as needed. LPN Qualifications : Education : Diploma in Practical NursingLicensure : Active LPN licenseRN Qualifications : Education : Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degreeLicensure : Active RN license Highly Preferred Skills : Proficiency with Epic EMR system.Experience working with geriatric populations.Timely and thorough patient triage skills.Computer competency (most of this position will be computer work).Excellent time management and prioritization.Adaptability in a new and changing environment.
