Kentucky Licensed Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care

Collaborate with providers and community resources to develop and monitor ongoing care plans

Provide education and support to members and their families regarding healthcare options and resources

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in case management preferred

Kentucky RN or Compact RN license required

Bilingual in English highly preferred

Experience with telephonic care management is a plus
