Oregon Licensed Clinical Program Coordinator

A company is looking for a Clinical Program Coordinator RN for a remote position. Key Responsibilities Provide care coordination services, including disease management and case management for members with complex health needs Assist members with managing health plan benefits and finding alternative services when needed Educate and empower members to manage their health conditions effectively Required Qualifications 5 years of clinical nursing experience Current unencumbered Registered Nurse License in state of residency upon hire Experience collaborating with physicians in patient care management Bachelor's Degree in Nursing or health education is preferred Current nursing experience in areas such as cardiology, endocrinology, pediatrics, obstetrics, oncology, or respiratory health is preferred
