Job Description : The Case Aide assists the Health Connect Caseworker through a variety of activities. This position will be daytime hours based on needs of program and participants. Up to 10 hours weekly.

Job Duties :

• Transports patients to medical appointments (primary care physician, specialists, diagnostic tests, lab tests, etc).
• Transports patients to appointments related to their clinical needs (individual counseling, group counseling, support groups, etc).
• Assists patients with housing stability by transporting them to appointments with potential landlords, Section 8 housing, etc.
• Assists patients with their financial issues (e.g. applying for social security, disability, unemployment, food assistance, etc.)
• Reports to the Caseworker regarding the status of planned patient transportation to appointments.
• Documents in case notes form their interactions with patients during transportation to appointments.
• Will make initial contact with potential new patients during periods when the Caseworker is busy working with existing patients.
• Participates in patient-related agency and hospital staffings and meetings.
• Assists the Caseworker with obtaining, and if needed, copying, medical records.
• Makes companionship calls to patients to check in with them regarding their health status and general well-being.
• Assists Caseworker in securing and linking patients to community resources.
• Performs other duties as assigned.
