Clinical Supervisor / Team Leader
Unity House of Troy is seeking a Clinical Supervisor / Team Leader to join our newest program the Critical Time Intervention (CTI) team. This exciting, evidence-based initiative supports individuals with severe mental illness as they transition from inpatient care back into the community. This is a management-focused position responsible for oversight and administration of the program, with minimal direct participant contact. This role oversees a multi-disciplinary team that partners with Mohawk Opportunities in Schenectady County, ensuring seamless collaboration and continuity of care. This role provides a unique opportunity for a clinical leader to make a lasting impact while helping to build and guide a new, innovative service model.
Key Responsibilities :
- Lead and supervise a multi-disciplinary Critical Time Intervention (CTI) team.
- Provide supervision and administrative oversight for all professional and paraprofessional staff.
- Ensure delivery of high-quality, evidence-based care consistent with CTI principles.
- Manage complex situations and competing priorities with sound judgment and leadership.
- Coordinate clinical treatment, psychosocial rehabilitation, and community support services.
- Collaborate with hospitals to facilitate discharge planning and community reintegration.
- Provide direct services as needed, including home visits and community-based interventions.
- Foster a culture of teamwork, inclusion, and mutual respect within the team and across agencies.
- Maintain effective collaboration with Mohawk Opportunities and other community partners.
Qualifications :
NYS Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) required.35 years of post-graduate experience working with individuals with mental illness.3+ years of supervisory experience, preferably leading a multi-disciplinary clinical team.Valid and clean New York State driver's license and reliable transportation required.Ability to work both collaboratively and independently in a mobile, community-based environment.Strong organizational, interpersonal, and problem-solving skills.Why Join Unity House?
