A company is looking for a Manager, Care Management.
Key Responsibilities :
Manage and develop a team of Team Leads and Care Coordinators to ensure high-quality patient-centric care
Oversee operational effectiveness and drive improvements in quality and key performance indicators
Act as a change agent to identify and implement process improvements across workflows and policies
Required Qualifications :
3-5 years of direct leadership experience in care management
Experience leading large remote teams through growth and change
3-5 years' experience in integrated Care Management or telehealth, preferably in Chronic Care Management
Experience in a healthcare setting, such as acute care or managed care
Bachelor's Degree in Nursing or Master's in Social Work preferred; active RN license is a plus
