Patient Experience Coordinator

We are seeking a dedicated and compassionate Patient Experience Coordinator to join our Certified Home Health Agency team. In this role, you will be at the heart of ensuring our patients receive outstanding and compassionate care. Your primary responsibilities will include following up with patients in the community to assess their satisfaction, addressing any concerns, and relaying important feedback to our clinical and quality assurance teams.

As a Patient Experience Coordinator, you will play a vital role in enhancing the overall care journey. You’ll work closely with our team to ensure that every patient feels heard, respected, and supported. Must-have experience : Candidates should have a background in customer service and / or hospitality, as these skills are essential in providing a warm and supportive experience for our patients.
