Responsibilities
The Diabetes Care and Education Specialist (DCES) 1 performs comprehensive management of blood glucose in patients with diabetes.
Provides collaborative, comprehensive and patient centered care, and education conducive to behavior change and improved quality of life.
Completes quality assessments and formulates individualized care plans based on patient assessed needs and treatment goals that reduce risks and optimize health outcomes, while supporting and promoting self-management.
Qualifications
Education
- Graduate of a Registered Nurse (RN), Registered Dietitian (RD) or Licensed Clinical Social Worker (LCSW) program requiredExperience
- Experience in direct patient care with diabetes patients preferred
- Bilingual English / Spanish preferredLicenses and Certifications
- CDCES - Certified Diabetes Care and Education Specialist required within 2 years of hire or transfer into position
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
- One of the following is requiredo RN - Current State of California Registered Nurse licenseo RD - CDR Registered Dietitiano LCSW - State of California Licensed Clinical Social Worker
