Sequoia Transitional Care , a beautiful and welcoming 99-bed skilled nursing facility, is hiring a dedicated LVN Charge Nurse for our Part-Time shift . Whether you're experienced or newly licensed, we offer a supportive environment, training, and growth opportunities to help you succeed.

Key Responsibilities :

• Provide direct nursing care to residents in accordance with individualized care plans
• Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and report to RN Supervisor or physician
• Document all care provided accurately and timely in EMR system
• Communicate effectively with residents, families, and interdisciplinary team
• Maintain compliance with safety, infection control, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, training, and care planning
