About The Position

Job Title : Case Manager

Experience : Minimum of 2 years in acute care hospital case management

License : Active Registered Nurse license

Certifications : Relevant nursing certifications

Must-Have :

• Previous charge experience
• Experience with care coordination, discharge planning, and using charting systems such as Epic
• Knowledge of Medicare and Medicaid services, including DRG (Diagnosis Related Groups)
• Ability to assess patients and develop treatment plans
• Competence in arranging DME, SNF, and ambulance transport independently

Description :

The role involves managing case loads and coordinating patient care effectively.

Experience in hospital case management is required.

Candidates must have skills in discharge planning and care coordination for various patient age groups, including adolescents, adults, and geriatrics.

Experience handling complex patient situations, including those with mental health issues, substance abuse, and housing instability is essential.

Participation in family meetings and organizational tasks is expected.

Traveling between facilities within the state may be required.

Flexibility to work every other weekend is necessary.

Candidates are expected to float between facilities as needed.

References from at least two supervisory roles covering the last three years in a similar specialty are required.
