  • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required
  • Proof of COVID-19 vaccination or declination required

  • Attach scanned copy of certifications with expiration dates to candidate's record in TrinityVMS
  • All engaged contractors must have page 27 signed form on file in Trinity VMS
