Post Acute Physician - Skilled Nursing Facility Physician

What You'll Do :

• Round at Skilled Nursing Facilities ' Manage chronic and acute conditions for patients in designated Orange County nursing homes.
• Reduce Hospitalizations ' Stabilize patients on-site and prevent unnecessary readmissions.
• Lead Goals-of-Care Discussions ' Partner with patients and families to ensure treatment aligns with their values.
• Drive Quality & Compliance ' Close care gaps, support value-based initiatives, and maintain excellent documentation.
• Collaborate & Lead ' Work closely with NPs, nurses, and care teams to deliver seamless, patient-centered care.
