We are currently seeking a dedicated Skilled Home Health Nurse (RN) to join our growing team and care for a ventilator and tracheostomy-dependent patient.
The Skilled Home Health Nurse delivers one-on-one skilled nursing care to a patient requiring ventilator and tracheostomy support in the home setting. This role focuses on respiratory management, medication administration, enteral feeding, Foley catheter care, and caregiver education ensuring patient safety, comfort, and stability while promoting independence for both patient and family.
Shift : 9 hours per night, 7 nights per week

Key Responsibilities :
Respiratory Management
- Monitor and manage mechanical ventilator settings, alarms and related respiratory equipment.
- Assess respiratory status (oxygenation, breath sounds, ventilator compliance) and promptly respond to changes or distress.
- Medication Administration
- Administer and document medications via prescribed routes (enteral, IV, other) following physician orders and agency protocols.
- Monitor for therapeutic effects, side-effects, and report changes in condition to supervising clinicians and physicians.
- Enteral Feeding Management
- Administer and monitor enteral feedings, ensure feeding tube patency, and hygiene.
- Assess tolerance (GI signs, aspiration risk, intake / output) and document accordingly.
- Foley Catheter Care
- Provide catheter care, monitor urinary output, signs of infection, and maintain documentation per protocol.
- Caregiver & Family Education
- Educate caregivers / family on ventilator and tracheostomy care, equipment handling, suctioning, infection prevention, emergency procedures.
- Documentation & Communication
- Accurately and timely complete all required clinical documentation.
- Communicate effectively with the interdisciplinary team, participate in case conferences, and support quality improvement efforts.
Qualifications :
Active and valid Florida RN or LPN / LVN license.Minimum 2 years of experience preferred in complex home care (ventilator / tracheostomy) or acute respiratory / critical care.Current BLS certification; ACLS preferred.Strong clinical assessment, critical thinking, and decision-making skills.Excellent communication and caregiverteaching skills.Ability to work independently in a home setting, adapt to changing patient status, and coordinate with home agency staff.Valid driver’s license, reliable transportation.
