Join KeystoneCare as an Intake Coordinator and become a vital part of our team dedicated to making a positive impact on patient care. In this role, you will be the first point of contact for patients and their families, guiding them through the referral process and ensuring they receive the care they need. Your work will involve collecting patient information, coordinating referrals, and collaborating with clinical staff to provide a seamless transition for our patients.
Key Responsibilities :
- Handle incoming phone calls and manage referrals from various sources.
- Provide information to patients and referral sources about our agency's services and programs.
- Assess referral eligibility based on location, clinical requirements, and insurance coverage.
- Input data accurately into our Electronic Medical Record system.
- Assign cases to clinical teams based on patient needs and resource availability.
- Ensure patient care begins in accordance with agency policies and guidelines.
- Maintain consistent communication with all stakeholders regarding patient status and care updates.
- Adjust clinical staff schedules as necessary for seamless coverage.
- Uphold patient confidentiality and comply with healthcare regulations.
- Take on additional duties as required.

Requirements
Minimum of 3 years experience in a healthcare setting, preferably in home health or hospice.Strong multitasking abilities in a fast-paced environment.Exceptional communication and customer service skills to interact effectively with healthcare team members, patients, and families.Highly accurate data entry with a solid understanding of patient scheduling.Self-motivated team player capable of working independently.Proficiency with electronic documentation systems and Microsoft Office Suite.Current proof of COVID-19 vaccination or an appropriate exemption required.Proof of flu vaccination or an approved exemption.Benefits
