RN Case Manager – 17-Week Contract | Albany, GA

Schedule : Days | 5x8s | 40 hrs / week

We are looking for a RN Case Manager to providing high-quality, patient-centered care coordination and discharge planning across inpatient units (Med / Surg, ICU, L&D, NICU, Mother / Baby, etc.).

Requirements : Active GA RN License

5 years total RN experience, with at least 2 years in hospital case management

UR and Care Transitions experience required

BLS certification

Must be proficient in complex discharge planning

EMR : MEDITECH 6.1

Details : 17-week contract

Orientation : 16 hours
