Job description

Licensed Social Worker (LSW, LISW) or Licensed Prefessional Counselor (LPC, LPCC)

Psycho Social Therapies is a leading mental health agency specializing in long-term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assist the residents transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and / or assisted living facility, ensuring that each client's unique mental health needs are met with compassion and expertise.

Responsibilities :

1. Evaluating Clients :

  • Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
  • Identify individualized support options based on these evaluations.

2. Creating Individualized Care Plans :

  • Develop customized care plans for each client, considering their unique circumstances and mental health requirements.

    • 3. Collaborating with Other Care Providers :

  • Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively.
  • Facilitate seamless communication and holistic care for clients.

    • 4. Monitoring Clients :

  • Regularly assess clients’ progress and adjust care plans as needed.
  • Keep track of treatment outcomes and make necessary modifications.

    • 5. Maintaining Records :

  • Document treatments, interventions, and services provided.
  • Ensure accurate and confidential record-keeping.

    • 6. Providing Talk Therapy :

  • Engage in therapeutic conversations with clients to address emotional and behavioral issues.
  • Use evidence-based techniques to support clients in coping with challenges.
