Position Summary
The Quality RN Work ow Specialist is responsible for ensuring the accuracy, consistency, and compliance of clinical documentation and work ows within the home health setting. This role supports quality improvement initiatives and regulatory readiness by leveraging Home Care Home Base (HCHB) to monitor, audit, and enhance clinical processes. The specialist collaborates with interdisciplinary teams to promote best practices and maintain high standards of patient care documentation.
Key Responsibilities
Clinical Work ow Oversight
- Monitor and evaluate clinical documentation work ows within HCHB to ensure alignment with agency protocols and regulatory requirements.
- Identify documentation gaps, inconsistencies, or delays and work with clinical staff to resolve issues.
- Assist in con guring work ow templates and task structures in HCHB to support efficient and compliant documentation.
- Collaborate with HCHB support and IT teams to address system issues and implement work ow enhancements.
Quality Assurance & Compliance
Conduct regular audits of patient charts, OASIS assessments, and care plans to ensure accuracy and completeness.Review documentation for adherence to Medicare Conditions of Participation, Medicaidguidelines, and accreditation standards.
Support survey readiness and participate in internal and external compliance reviews.Maintain HIPAA compliance and promote secure documentation practices.Education & Staff Support
Provide training and ongoing education to clinical staff on HCHB documentation standards and work ow protocols.Develop and update training materials, audit tools, and reference guides.Serve as a resource for clinicians regarding documentation best practices and work ow troubleshooting.Facilitate feedback sessions and performance reviews related to documentation quality.Interdisciplinary Collaboration
Partner with nursing, therapy, and administrative teams to ensure documentation supports coordinated, patient-centered care.Participate in case conferences and contribute to care planning and quality improvement discussions.Communicate ndings from audits and work ow reviews to leadership and recommend corrective actions.Quali cations
Education & Experience
Registered Nurse (RN) required.Associate's or Bachelor's degree in Nursing or related eld required.Minimum 2 years of experience in home health clinical documentation and quality assurance.Prior experience with Home Care Home Base (HCHB) is preferred.Strong knowledge of OASIS, Medicare Conditions of Participation, and home health regulations.Skills & Competencies
Detail-oriented with strong analytical and critical thinking skills.Pro cient in HCHB navigation, reporting, and work ow con guration.Excellent communication and training skills.Ability to manage multiple priorities and work independently.Collaborative mindset with a commitment to continuous improvement.Physical Requirements
Prolonged periods of sitting and working on a computer.Occasional lifting of up to 15 pounds.Frequent use of hands, hearing, and vision for computer and phone tasks.Additional Information
