Role Description

We are seeking compassionate and motivated Family Nurse Practitioners (FNPs), Physicians (MD / DO), and Foreign Medical Graduates (FMGs) to join our growing Home Health team. This role focuses on delivering personalized, high-quality medical care to patients in their homes.

Key Responsibilities :

  • Conduct in-home medical evaluations and follow-up visits
  • Develop and manage patient care plans in coordination with the clinical team
  • Diagnose, treat, and manage acute and chronic conditions
  • Educate patients and families on treatment plans and preventive care
  • Maintain accurate and timely clinical documentation
  • Collaborate with multidisciplinary teams to provide comprehensive care

Qualifications :

  • Licensed FNP / MD / DO in the State of Illinois (or eligibility to obtain a license)
  • FMGs with clinical experience are encouraged to apply
  • Prior experience in home health, internal medicine, or primary care preferred
  • Strong clinical judgment, communication, and organizational skills
  • Compassionate and patient-focused approach
  • Ability to travel within the greater Chicago area and nearby cities
