Job Title : Physical Therapist - Home Health

Job Specialty : Rehabilitation Therapy

Job Duration : 13 weeks

Shift : Days

Guaranteed Hours : 40 hours per week

Experience : Minimum of 1 year in Home Health

License : California PT License

Certifications : N / A

Must-Have :

  • One year of Home Health experience

Job Description :

  • Required to prioritize start of care visits
  • Supervise Physical Therapy Assistants
  • Mandatory next-day reporting of start of care visits
  • Clinical notes and other documentation due within 48 hours of visit
  • Maintain timely communication with supervisor, intake, and office
  • Be generally responsive during normal business hours
  • Coverage area includes San Francisco
  • Schedule includes working Monday through Friday and one weekend a month rotation
  • Training consists of three full days : one in-house and two in the field
  • Utilize Kinnser Documentation System
  • Equipment provided includes iPad and Verizon One Talk
  • Mileage reimbursement is provided
  • Opportunity for company car with adjusted terms
  • Productivity requirement of 30 points per week
  • Caseloads and visit mixes per day will equate to an average of 8 hours per day and 40 hours per week
  • Pre-approval required for extended working hours
