Registered Nurse Chronic Care Program Manager

As the Registered Nurse Chronic Care Program Manager , you will lead and implement chronic care case management programs within our organization, specifically targeting patients at the highest risk. In this role, you'll design and oversee chronic care management models for Medicare and high-risk populations and guide the care management team. Your expertise will drive positive change, enhance patient care, and contribute to our mission of improving the health of our patients. This position is based in Yakima, WA, and is not remote. A relocation allowance is available.

Must have experience in care coordination, case / care management, utilization / resource management, quality management, and / or transition of care, and supervisory nursing experience.

What We Offer

• Sign-on Bonus $7,500 in first paycheck, $2,000 at 12 months.
• Salary $89,000 -$119,000 DOE with ability to go higher for highly experienced candidates.

Qualifications

Registered Nurse (RN) with current licensure to practice in Washington State or hold a valid Multi-State RN license.

Must submit proof of license in the state of residence.
