As a Certified Nurse Aide for Homeland Hospice, you would contribute to our community of caring by providing direct support to patients and family members during a significant part of their life journey.

As Homeland Hospice's CNA you would provide personal care and light housekeeping services to terminally ill patients in their home, nursing facility, inpatient units or other environments where the Hospice patients reside, in accordance with an established plan of care and under the supervision of a Registered Nurse (RN). You would actively participate as a member of the Hospice Interdisciplinary Team (IDT) and provide care and comfort to patients with assigned tasks following the established IDT and Hospice Aide plan of care. You would observe the patient, report and document observations and care performed and provide timely documentation of services based on the patient's care plan.
CNA Requirements include :
Minimum age requirement of 18.

Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under Medicare § 483.151 through § 483.154 and is currently listed in good standing on the State nurse aide registry.
or
Completion of a state approved training program and certification as a home health aide, and proof of certification.
Valid Pennsylvania CNA Certification.
