Experience :

Minimum of 2 years of acute medical care management experience

Travel experience required

California state license required; pending license accepted but must be licensed upon start date

Certifications :

Current certifications relevant to case management and nursing

Must-Have :

Experience with assessing patients

Experience in care coordination and discharge planning

Ability to organize and participate in family meetings

Experience with HMOs and IPAs

Knowledge of adoption / foster care and drug / alcohol abuse treatment options

Awareness of resources for patients experiencing homelessness and those with mental health histories

Competence in arranging durable medical equipment, skilled nursing facilities, and ambulance transport

Experience with computerized documentation

Description :

This position requires the management of patient care to ensure effective care coordination.

The case manager will work with adult and geriatric patients, coordinating treatment plans and facilitating care transitions.

Must be able to evaluate patient needs and develop individualized care plans.

The role includes participation in interdisciplinary meetings and collaboration with healthcare professionals to enhance patient outcomes.

A commitment to maintaining compliance with health regulations and standards is essential.

The case manager will ensure appropriate discharge planning is conducted for each patient.

Flexibility to float between facilities may be required based on staffing needs.
