Location is Northwest OH, The expectation is 2-3 days in the facility and the rest of time work from home for documentation needs.

Shift : Mon - Fri 8-5pm (1 hr. lunch)

Duration : 6 months, possible extension

Job Description : The daily schedule is 8-5pm. The schedule can be self-managed based on visit schedule. The expectation is 2-3 days in the facility and the rest of time work from home for documentation needs. Long term care behavioral health members have specialized needs. Develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.

Must haves : 2-4 years of Behavioral Health experience

Nice to haves : Master preferred, LTC experience, CCM

Disqualifiers : no licensure
