Bayhealth, Inc. is hiring a Per Diem Certified Home Health Aide. Responsibilities include providing personal care services, observing patients, and documenting care.

Certified Home Health Aide Responsibilities :

Provide hands-on personal care such as bathing, oral hygiene, and changing bed linen.

Assist patients with dressing, toileting, and skin care.

Maintain cleanliness and orderliness of the patient’s living area.

Plan and prepare nutritious meals, and assist in feeding if necessary.

Take and record vital signs when ordered.

Aid in ambulation, exercise, and range of motion exercises as directed.

Assist with medication administration and laundry.

Ensure patient safety and proper equipment usage.

Report patient condition and changes to the assigned nurse.

Collaborate as part of the interdisciplinary team.

Adhere to organization's documentation and care procedures, and uphold professional standards.

Qualifications :

Preferably registered on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau.

Minimum age requirement of 18.

Proficiency in reading and following written instructions, and documenting care.

Ability to work independently with minimal supervision.

Demonstrated empathy for the needs of patients.

Current CPR certification.

Exhibit tact, patience, and good personal hygiene.

Possession of a valid driver's license, insured vehicle, and good working condition.

Strong prioritization and multitasking abilities.

Excellent organizational, time management, and customer service skills.

Proficiency in communication and computer usage.

High attention to detail.
