Physical Therapist - Home Health | Full-Time PT Position in Lancaster

• Monitor and document patient progress, outcomes, and response to treatment using electronic medical records
• Adjust and modify treatment plans based on patient progress and changing needs
• Ensure compliance with all federal, state, and agency regulations governing home health physical therapy services
• Manage caseload efficiently while maintaining flexible scheduling for home visits throughout the Lancaster service area
