About the Role
1st Choice is seeking detail-driven Document Imaging Techs / Medical Records Clerks to support a Corporate HIM Operations. You’ll retrieve charts, prep/scan/index documents into the EMR, perform QA, and help ensure timely, accurate, and compliant medical records—work that directly supports patient care and revenue integrity.
Responsibilities
Retrieve, prep, scan, and QA patient records per HIM Prep/Scan Guidelines.
Index documents to the correct patient/encounter (CSN), document type, and location; split/merge, append/insert pages per UMMS indexing rules.
Identify poor-quality images and rescan as needed; verify orientation/legibility.
Work assigned electronic worklists daily and process items on time.
Death certificate processing: validate completion/submission per state regs; coordinate with physicians and funeral homes as needed.
Provide Release of Information coverage as assigned.
Meet and report weekly productivity and quality standards; assist with training new staff.
Communicate issues promptly to the Team Lead/leadership.
Requirements
HS Diploma/GED.
1+ year document imaging in a high-volume hospital HIM setting (indexing experience required).
Proficient with Microsoft Suite; comfortable working in EMR imaging/indexing workflows.
Ability to walk ?40 minutes/day and carry up to 20 lbs.
Strong accuracy, organization, and turnaround; clear, professional communication.
