We are seeking an RN Case Manager to join our team! You will be responsible for the assessment, care plans, and documentation of assigned residents.
Position : Assessment RN
Location : Sacramento CA
Conduct initial and monthly assessments on residents in assisted living facilities / Skilled Nursing facilities
Pay rate : $75 / assessment (20 mins)
Responsibilities :
- Visit - residents at a local assisted living facility, monthly
- Monthly assessment, semi-annual comprehensive assessment with a care plan
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians, administrators, and family members regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to residents and their family members
- You are in charge of your schedule; flexibility is yours
Qualifications :
- Previous experience in nursing and / or case management fields
- Familiarity with medical software, office suites / excel, word
- Ability to build rapport with residents and referral sources
- Strong problem-solving, confidence in your nursing abilities
Requirements
- Must have an active nursing license in the state of CA- in good standing
- Experience in Home Health, Geriatric, and Long-term Care a plus
- Current BLS
- Current TB Skin test
- Must have experience with office suites - Excel / Word / PDF / Adobe
- Two current references
- Must be able to pass a background screening
