Geriatrics physician job in Florida : Permanent Geriatric Medicine Medical Director Pensacola, FloridaSetting : Multidisciplinary Care Model (includes community-based settings)Visa Sponsorship : Not offered Position Overview :

Conduct H&Ps on new participants and perform event-based or periodic reassessments

Develop medical treatment plans and provide medically necessary services across participant care settings

Participate in intake and assessment evaluations with applicants and caregivers

Discuss prognosis and care plans with participants and families to support shared decision-making

Collaborate across nursing and interdisciplinary teams on treatment planning

Actively contribute to team / family conferences and medical decision-making

Educate patients and families on chronic conditions and health maintenance

Supervise care plans developed by the interdisciplinary team

Order and evaluate labs / diagnostics, document results systematically

On-call responsibilities as required Practice Environment :

Office-based leadership role within a collaborative, interdisciplinary model

Nursery or inpatient pediatric care not applicable

EMR and documentation systems are in place

Organization promotes a culture of teamwork and shared goals across all divisions Requirements :

Board Certified or Board Eligible in Geriatric Medicine, Internal Medicine, or Family Medicine (typically expected)

Prior experience in geriatric or complex patient care preferred

Must be able to work collaboratively within an interdisciplinary team
