Location : This position is located in Portland and will require travel throughout York County.

Core Responsibilities :

The Community-based Intensive Case Manager(s) will :

• Provide professional services work in intensive case management for clients in the community who are at-risk of disrupted services or at-risk of involvement in the Criminal Justice System.
• Advocate for and / or assist clients in gaining access to and maintain medical, psychiatric, self-help, rehabilitation, social, educational, psychological, residential, financial, and other necessary support services.
• Co-respond with and / or respond upon referral from crisis providers and / or law enforcement to assist clients immediately post crisis to augment or maintain necessary community services.
• Appropriately confer with clients, professional medical / clinical personnel, providers, and other concerned parties to identify unmet needs and necessary supports / services and develop a plan of assistance based on clients' needs, preferences, circumstances, and team assessments.
• Assist clients in accessing and maintaining needed services to support clients' community integration efforts and plan.

Proactively outreach key stakeholders to increase familiarity and anticipate needed services.

Minimum Qualifications :

An Associates Degree in one of the Social or Behavioral Sciences and 4 years' experience in professional level mental health work -OR- a Bachelors Degree in one of the Social or Behavioral Sciences and two (2) years' experience in professional level mental health work -OR- a Master's Degree in one of the Social or Behavioral Sciences. Equivalent work experience in professional level mental health work may be substituted for education on a year-for-year basis.

Candidates must have or be eligible for a Mental Health Rehabilitation Community Certification (MHRT / C) as issued by the Muskie School of Public Service or LSW.

Our role is to ensure Mainers with mental health, substance use, and co-occurring disorders are not just symptom free but living a life of dignity, hope, and meaning. We partner with providers, communities, and citizens to offer support, resources, education, training, technical assistance, and services.
