Physical Therapist - Home Health - (PT - HH)

Pay Rate : $2120.34 - $2323.35

MUST HAVE HOME HEALTH EXPERIENCE

MUST BE ABLE TO GET MAINE LICENSE

Required Certifications (will this be consistent for all similar skillset on unit or specific to this Position Request) : Valid Maine Physical Therapy License. BLS / CPR

Years of Experience : 3

Call Requirements : No

Weekly Guaranteed Hours : 36

Shift : 8-4 includes some weekends with comp days

Required Qualifications

Physical Therapist, Home Health

Experience : 3 years

Licenses : PT-ME

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year

References : 2 References in entire work history
