Key Responsibilities

Support the behavioral health needs of patients through evidence-based treatment and assessments

Collaborate with prescribing providers for psychotropic medication management and monitor treatment adherence

Document patient encounters and participate in clinical team meetings to communicate patient needs effectively

Required Qualifications

Master's degree in Social Work

Current California independent licensure required

2+ years of prior healthcare / behavioral health experience required

Geriatric care experience highly desired

Proficient in computer skills, including Microsoft Office Suite and EMR experience
