Position Summary

We are seeking an experienced Registered Nurse Case Manager to join the dynamic healthcare team. You will function as part of an interdisciplinary team to support optimal patient outcomes and smooth transitions of care.

Requirements

Accredited nursing degree required.

If degree is not accredited, 5 years of Case Management experience required.
