Camino Ridge Post Acute is a 102-bed skilled nursing and rehabilitation facility located in Mountain View , California Address : 1949 Grant Rd, Mountain View, CA 94040 Key functions of SNF physical therapy Evaluation and assessment : Physical therapists assess a patient's safety, mobility, and current level of function upon arrival. Treatment planning : They create individualized treatment plans that may include exercises, stretches, and other therapeutic techniques like aquatic therapy. Restoring function : Therapists work to improve strength, coordination, balance, and range of motion. Pain management : Treatments are used to reduce pain and inflammation. Preventing complications : A key goal is to prevent falls and hospital readmissions. Education and communication : Therapists educate patients about their care plan and communicate with the entire care team, including nurses, other therapists, and doctors, to ensure a coordinated approach. Transition planning : Physical therapy is crucial for helping patients prepare to return home or to a new living situation. Long-term care : Therapy is also used for residents with chronic conditions like arthritis or Parkinson's disease to help them maintain function and independence. Pay Packages are all inclusive, built on 40 HRS / 13 weeks and subject to confirmation and other fees not defined by facility.
