A company is looking for a Care Manager Bilingual in English and Korean - 100% Remote.

Key Responsibilities

Advocates and educates beneficiaries on services and self-management techniques

Conducts assessments and develops care plans in alignment with treatment goals

Coordinates care among providers and documents all interactions in the appropriate systems

Required Qualifications

NYS RN or LCSW or LMSW (any state)

Fluency in Korean

For Behavioral Health positions : 3 years of work experience in a mental / behavioral health or addictions setting

1 year of pediatric clinical field experience for PEDS positions

Experience managing member information in a shared network environment
